Free & Reduced Lunch Application - 2011/12
* Print, Complete, and Mail/Fax/Email back a hard copy of this form to the REALMS office.  This form must be re-submitted every year.  Please do not file electronically through the Bend La Pine school district.

Volunteer Background Check Form
* complete (and send, email, or fax to us) this if you are interested in volunteering at REALMS. The form is valid for 3 years.
